Availability of Form 1095-C
To comply with IRS reporting requirements under the Affordable Care Act (ACA), the city prepares Form 1095-C, which provides information about the health coverage offered to you as a full-time employee, and non full-time employee.
The IRS no longer requires employers to automatically mail Form 1095-C to employees; however, your form 1095-C is available to you upon request at any time. If you would like to receive a copy of your Form 1095-C; please contact:

Please include your full name and current mailing address with your request. Your Form 109-C will be provided within 30 days of receiving your request. This form is not required to file your federal tax return, but you may wish to keep it with your tax records.
